The Growing Chronic Disease Epidemic
The United States is losing the war against preventable chronic disease, and America is getting sicker with each generation. 84% of healthcare costs fund the treatment of chronic disease while the outcomes worsen. Neither our health system, American families, nor our economy is equipped to defend against it. Culturally, Americans have accepted chronic disease as the norm.
The Racial Health Gap
Racial health disparities are an urgent and unsustainable crisis for families, cities, and the health industry. The US has one of the largest racial health gaps in the world. People of color are both the fastest growing and the sickest patient population in the US. Yet it is one that the health sector does not know how to effectively reach. Widening racial health disparities and changing demographics will only exacerbate the chronic disease epidemic and its economic burden.
Empowering Patients
In today’s environment, patients must manage their own health. Black & brown patients in particular are in critical need of health partners they can trust as they navigate the crisis of chronic disease.
Bridging the Trust Gap
People of color have different health profiles, different cultural norms, and a distinct historical relationship with the US healthcare system. Many health organizations don't have the bandwidth to develop and execute culturally resonant health messaging to these audiences that they need to reach the most.
We help ensure your health promotion investment not only gets to your target audience, but breaks through to the communities you are trying to reach. Transcend bridges the trust gap between healthcare and medically underserved communities of color.
